This deals with clubs who wish to carry out licensable activities, for example The Royal British Legion.
Application for a Club Premises Certificate

This application form is required when a qualifying club wishes to carry out licensable activities like:
- The sale by retail of alcohol
- The supply of alcohol by or on behalf of a club to, or to the order of, a member of the club
- The provision of regulated entertainment, which include:
- A performance of a play
- An exhibition of a film
- An indoor sporting event
- A boxing or wrestling entertainment (indoor and outdoor)
- A performance of live music
- Any playing of recorded music
- A performance of dance
- Entertainment of a similar description to that falling within the performance of live music, the playing of recorded music and the performance of dance
- The provision of late night refreshment
Application to vary a Club Premises Certificate Word 685K
This application form is required when a qualifying club who is already licensed wishes to vary any of the times and or their licensable activities.
Declaration for a Club Premises Certificate Word 149K
This form is required when a club is applying for a Club Premises Certificate enabling them to state that they meet the criteria to be a qualifying club.

Reviews of Premises Licences and Club Premises Certificates
The application form is required by anyone wishing to seek a review of a licence that has been previously granted under the Licensing Act 2003.
A review must be based on 1 or more of the licensing objectives these are:
- Prevention of crime and disorder
- Prevention of public nuisance
- Protection of children from harm
- Public safety
