Our aim in the Adult and Community Services Department is to provide the support needed to enable people with particular needs to live in the community as independently as possible.
There is a range of services available to residents in the borough who are 18 or over and need help with their day to day care needs. You may be eligible for services if you are:

Not everyone who asks us for help can be given a service because we must make sure we can help those in greatest need. In order to obtain services, you will have to meet our eligibility criteria.
If we cannot offer a service, we can give information and advice about other organisations that may be able to help.
Once a referral is accepted you will be contacted by a member of our team who will talk to you about what sort of help you might need. They may also talk to anyone who currently helps you at home. This will lead to an assessment, which will help us to decide which services we can arrange for you.
If you or someone you know may need support from the services we provide, you should contact the Adult Intake and Access Team by telephoning 020 8227 2915 or by completing our online request for an assessment form.
Alternatively, the Adult Intake and Access Team are based on the ground floor of the Civic Centre annexe building. You may write, telephone or go directly to the Social Service Reception yourself, or a relative, neighbour, friend, advocate or someone you know may do this for you. You may have been referred to the Adult Intake and Access Team as needing support by your GP District nurse or other health professional. If you are in hospital, and will need support on discharge, the nursing staff should contact us before you are discharged.
We do not charge you anything for the assessment or any information and advice we may give you. However, there may be charges for some of the services we provide. This however, will be fully explained to you by a member of our team.
