Information on Care Plans for people receiving services

 

What is a care plan?

After your assessment we will discuss with you your options and write down any arrangements in a care plan. Your care plan will describe:

Services will not always be provided direct from us. Sometimes we arrange for private or voluntary agencies to provide services on our behalf.

 

Will you have to pay?

We do not charge you anything for the assessment or any information and advice we may give you. However, there may be charges for some of the services we provide. This however, will be fully explained to you by a member of our team.

 

Charges will vary. There is no payment for those people on very low incomes and no savings. Those with a higher income and/or savings will have to pay up to a maximum amount.

We will always respect and try to meet your language, cultural and religious needs when assessing or arranging services. If you would like to talk to us about how we may be able to help you, please contact Adult Intake and Access Team at the Civic Centre.
