The complaints leaflet is a guide and complaints form that explains to you, what you can complain about, how you can go about complaining and what happens after you make a complaint.

 

The young people who attend 12-16 Foster group designed this complaint form.

 

If you would like a complaints form you can contact your social worker or if you do not want to do that you can contact Kevin Donovan on 020 8227 5942 or Linda Smith on 020 8227 2405 / 2111 and they will send you a copy.

 

Alternatively, you can fill out the online complaints form which will be sent to a complaints officer at Social Services.

 

If you don't want to fill out a complaints form to make a complaint, you can call the freephone complaints line and they will make a note of your complaint and try and sort it out for you.
