If you or a member of your family are leaving hospital it may be that you may require extra support at home. We aim to provide a number of services that promote fast recovery from illness and support independent living.
Adult and Community Services can offer information, advice and assistance through our Adult Intake and Access Team based at the Civic Centre in Dagenham.
Benefits and bills can offer further advice and assistance on your benefit entitlements during your stay in Hospital.
If you are in hospital and have continuing support needs, we will ensure that you leave hospital without delay and that your care needs following discharge are met. Our aim to make it easy for you to get the support that you need.
Help that may be available to you could include:
It may be that on leaving hospital, you may need to go into a residential or nursing home. This could be temporary, to give you time to recover more fully, or to go get services started for you at home, or it might be permanent. Our staff can help arrange this, or give you advice if you are able to make your own arrangements.
We will not recommend that you move into residential or nursing care without first discussing with you all the available options that might allow you to remain in your own home. These options may include rehabilitation, intermediate care, recovery services or sheltered housing.
