Report Fraud 

Please provide as much information as you can to enable any investigation to be completed successfully. All information you provide will be treated in the strictest confidence.

Use the following boxes to give details of the fraud itself. You should say what your concern is; whom is involved; why you are concerned; when did the concern arise; if there are other people who can verify your concern. Please include such information as dates, times, places, important reference numbers, people you have already spoken to or people we should speak to regarding the fraud.

Details of person committing fraud

  1. What is their name? Are they known by any other name, a nickname maybe? Please give us their name and/or any other names that they may be known by.

     

  2. Do you know where they live? Please give us the address, including the town, county and postcode. Can you describe their property?

     

  3. Do you know where they work? Please give us the address, including the town, county and postcode. Please give details of their position/role if known.

     

  4. Do you know their phone number, mobile or landline? Do you know their email address? Please give details.

     

  5. Does the person involved own a car or other vehicle? Please provide details such as the make, model and registration number. The colour of the car or vehicle. Does it stand out in any way? For instance rims, tinted windows, special paint job, etc.

     

  6. Can you describe the person involved? It would be helpful to have a description of the person. Useful details to tell us include the person's race, age or date of birth, and sex. What is their height and build? What is their hair like? Do they have any distinguishing marks, a birthmark or scar? Can you describe any tattoos they might have and where?

     

  7. Who do they know? Are there other people involved? What are the names or nicknames of the people they know? What is their involvement? Can you tell us their address, including the town, county and postcode. What do they look like? Include race, age, sex, height and build. Do you have a phone number for them?
