This includes help around behaviour management, implementing routines, completing and reviewing care packages, transition into adult services and access to specific services that require professional referral and intervention.
We receive referrals through a variety of sources including multi agency panels – MAPs, multi agency safeguarding hub – MASH, schools, external agencies and through cases stepped down from social care.
Available for parents/carers looking after children with additional needs.
