New Application

New Licence Applications require the following to be submitted:

1. A completed Application for a New Special Treatments Premises Licence form (PDF, 241 KB)

2. A completed practitioner details form (PDF, 116 KB) for each proposed practitioner

3. Copies of qualifications for each practitioner in respect of each treatment they propose to provide, a copy of photo identification for each proposed practitioner and a separate digital photo

4. A completed Declaration of Convictions form (PDF, 128 KB) for all persons and/or organisations named on the application form

5. Plan or line drawing of the premises (preferably no larger than A4 size with a minimum scale of 1:100), which should identify the following:

• the full extent of the proposed licensed premises
• defined treatment areas/rooms
• entrances, exits and escape routes
• WCs, showers and wash hand basins
• passageways
• general location of equipment
• safety equipment including: - emergency lighting - automatic fire detection - firefighting equipment - Fire Exit signage. In addition, 

6. Where the proposed special treatments include laser and or and/or intense pulsed light (IPL) treatments: 

• a copy of a contract for the services of a Laser Protection Advisor (LPA), who must be certified and registered with either RPA 2000, the Association of Laser Safety Professionals or Public Health England
• evidence that the LPA has visited the premises and undertaken an assessment of its suitability for use for the provision of laser/IPL treatments
• details of the make, model, serial number and technical specifications of each item of laser/IPL equipment

Renewal of a licence 

Licence Renewal Applications require the following to be submitted: 

1. A completed Application to Renew a Special Treatments Premises Licence form (PDF, 151 KB)

2. If any changes to the licence are required, a completed Application to Vary a Special Treatments Premises License form (PDF, 228 KB) and associated documents must also be submitted.  Please see our Making changes to an existing licence page.
